Auto-authentication of patient records entries in paper-based environment - case study of General Hospital, Lagos Island

Abstract
Authentication is an attestation that something such as a medical record is genuine. The purpose of authentication is to show authorship and assign responsibility for an act, event, condition, opinion, or diagnosis (AHIMA, 2011). Every entry in the health record should be authenticated and traceable to the author of the entry. The Rules of Evidence indicate that the author of the entry is the only one who has knowledge of the entry. This is also known as Auto-authentication or Auto-attestation. Auto-attestation is the process by which an entry is implicitly signed (attested) through the user authentication process at sign-on. It is also the process by which a physician or other practitioner attests to an entry that he or she cannot or has not reviewed because it has not yet been transcribed or the electronic entry cannot be displayed (CMS, 2014). A system of auto-authentication in which a physician or other practitioner authenticates an entry that he or she cannot review, e.g., because it has not yet been transcribed, or the electronic entry cannot be displayed, is not consistent with these requirements. There must be a method of determining that the practitioner did, in fact, authenticate the entry after it was created. In addition, failure to disapprove an entry within a specific time period is not acceptable as authentication. The study used a descriptive methodology in which two hundred questionnaires were administered to respondents. It was only shown that only 6.5% of respondents have knowledge of auto-authentication but all respondents understand the meaning and importance of authentication.
