Abstract
Prostate cancer (PCa) is a global health burden with risk factors including race, age, genetics, environment, and lifestyle. PCa exhibits diverse phenotypes from indolent to aggressive life-threatening forms. Early detection enables better outcomes, but historic tumor markers lacked adequate sensitivity and specificity until prostate specific antigen (PSA). However, PSA does not reliably distinguish aggressive cancers. Thus the Gleason and subsequent International Society for Urological Pathology (ISUP) grading systems emerged to categorize tumors by aggressiveness. However, biopsy and treatment carry complications that may outweigh the benefits for latent tumors. Ongoing research seeks markers to predict PCa aggressiveness. Serum testosterone (T) shows promise but studies conflict regarding its influence on PCa development and progression. This hospital-based cross-sectional study will correlate pre-treatment T levels with ISUP grade among 72 purposively sampled PCa patients at Nnamdi Azikiwe University Teaching Hospital, pending ethics approval and informed consent. The Statistical Product and Service Solutions version 23 will analyze the results. The study will elucidate the association between T and PCa aggressiveness in this understudied population. If T links to high-grade tumors, it could serve as an affordable predictor of patients needing more aggressive management, preventing overtreatment of indolent cases.
