Abstract
Monitoring and evaluation are qualitative tools that detect common errors impeding program success and relevance to healthcare by assessing work performance. Monitoring and evaluation are critical for healthcare delivery as they indicate a country's health status, guiding intervention and care distribution. This study aimed to determine monitoring and evaluation's impact on Nnamdi Azikiwe University Teaching Hospital, Nnewi and explore the consequences of improper health information documentation and maintenance in healthcare delivery. A descriptive survey design thoroughly investigated the problem by identifying monitoring and evaluation characteristics, frequencies and categories in healthcare delivery. The hospital's head of department indicated 62 licensed Health Records Officers across units. Data collection relied on questionnaires, observation and interviews serving as the primary evidence basis.  All 62 respondents agreed that monitoring and evaluation impact healthcare delivery and have qualified personnel, while 77.4% noted problems and 22.6% disagreed. The Chi-square test result, 10.2, exceeded the 3.841 tabulated value at a 0.05 significance level with 1 degree of freedom. Thus, the alternate hypothesis was accepted, rejecting the null. Recommendations include employing qualified, trained Health Information Management professionals to utilize monitoring and evaluation and providing seminars, workshops and periodic trainings to increase staff effectiveness and efficiency....
