This project work examined the factors militating against effective documentation in health care delivery system, a case study of University of Benin Teaching Hospital in Edo State. The objectives are to determine the attitudes of health information professional/personnel on quality documentation of patient’s health record in University of Benin Teaching Hospital, to assess the knowledge of health information professional/personnel on quality documentation of patients health records in University of Benin Teaching Hospital in the study area. Many allied literatures were reviewed to develop more facts for the project work. This research use descriptive survey to sample 100 youth residents of University of Benin Teaching Hospital. Questionnaire is the instrument used to gather and analyze the data, 100 questionnaires were distributed out of which 100 were retrieved. In conclusion, recommendations were made that the chief record officer should delegate responsibilities to staff adequately trained in protocol and good clinical practice, government should try and employ more health information professionals, provision of storage facility for case files to avoid misfiling and health information professional should be well enumerated to avoid strike, there should be provision of adequate materials for proper documentation and government should organize more seminars on proper documentation of patient health record for health information professionals/personnel.
