This study examined the challenges faced by Nnamdi Azikiwe University Teaching Hospital (NAUTH) Nnewi and Chukwuemeka Odumegwu Ojukwu University Teaching Hospital (COOUTH) Amaku in managing health records libraries and ensuring quality health information. The background highlighted the importance of accurate records, data security, and proper organization for informed decision-making and continuity of care. The study adopted a descriptive survey research design, with a sample of 121 respondents selected using Yaro Yamane’s formula from a population of 173 health records staff across both institutions. Data was collected through a structured questionnaire and analyzed using simple percentages and chi-square tests. The research tested the hypothesis that there are no significant challenges in health records management and information practices at NAUTH and COOUTH. Results indicated overwhelming agreement among respondents on the essentiality of health records management (93%), the potential of electronic records (85%), and the importance of data privacy (80%). However, key challenges identified included limited physical storage space, inefficient retrieval processes, data inaccuracies, lack of interoperability, and training deficiencies. The chi-square test (2 37.7228) led to the rejection of the null hypothesis, confirming significant challenges exist.
