It evaluated the level of awareness of health records linkage among the health records professionals in the hospital. The study examines the severity of the challenges affecting the effectiveness of health records linkage in the hospital. The research was conducted using a survey kind of research which gives room for effective studying of the population. The instrument used to gather data for this study is a structured questionnaire having two (2) sections (A-B) with 25 items. A stratified random sampling technique was used to select the sample for this study to ensure that equal opportunity was given to the medical records personnel the data was analyzed using simple percentage. A total of 110 questionnaires were distributed and all were retrieved. The findings showed that the majority of the health records personnel have a good knowledge of health records linkage and that deterministic linkage is the predominant linkage system in the hospital. Furthermore, the analysis showed that inadequate internet connectivity, software limitation, lack of ongoing training and development, and resistance to change. Also, the findings showed that these challenges severely affect the effectiveness of health records linkage. This study concluded that the health records linkage system at UNIMEDTHC has both strength and weaknesses. While it is deemed effective by most personnel, significant technological, infrastructural, and human resources challenges hinder its optimal performance.
