Despite their critical role, these practitioners face numerous constraints that hinder their ability to deliver optimal services. Key issues include technological challenges, such as outdated systems and interoperability problems, which lead to inefficiencies and potential errors in patient records. To carry out the study, the following objectives were set: Identifying the method of record keeping being practiced and the roles of health records practitioners in UCH Ibadan University College Hospital Ibadan. To find out the challenges facing health records practitioners in UCH Ibadan University College Hospital Ibadan, to ascertain if there is the existence of misfiling patient case notes in the health records department, in UCH Ibadan University College Hospital Ibadan. From these objectives, research questions were formulated to enhance accurate research outcomes. Most relevant literature works done on the subject under investigation were reviewed. The study population was made of 125 respondents (HIM practitioners) who were selected from University College Hospital Ibadan. Nigeria. The instrument employed was a structured questionnaire personally distributed by the researcher and all questionnaires were retrieved.
