Impact of national health insurance scheme on healthcare services in State Specialist Hospital Benin City, Edo state

Abstract
This study examines the impact of the National Health Insurance Scheme (NHIS) on healthcare service delivery at the State Specialist Hospital, Benin City, Edo State. The research evaluates the scheme’s influence on patients’ access to care, affordability, service utilization, and overall quality of healthcare. Although NHIS has improved access nationwide, challenges such as low enrollment, financial constraints, and administrative inefficiencies continue to limit its effectiveness. The study is anchored on the Health Belief Model, which explains health-seeking behavior, and Systems Theory, which provides the main framework by highlighting the interdependence of patients, providers, institutions, and financing mechanisms within the healthcare system. A mixed-method approach was employed, using hospital records, structured questionnaires, and interviews with patients and healthcare providers. A total of 152 respondents were selected through probability and non-probability sampling techniques. Findings indicate that NHIS has lowered financial barriers to care, encouraged timely hospital visits, increased utilization rates, reduced catastrophic health expenditures, and enhanced patient satisfaction with service affordability. Nonetheless, critical challenges persist, including inadequate service coverage, limited public awareness and enrollment, bureaucratic bottlenecks, and delays in provider reimbursement, which undermine the scheme’s efficiency. The study concludes that while NHIS has positively influenced healthcare delivery in the hospital, its sustainability and effectiveness depend on stronger policy commitment, efficient management, broader service and population coverage, and enhanced public sensitization. Addressing these gaps will enable NHIS to achieve its mandate of providing equitable and accessible healthcare for all Nigerians....
